Psychological Services

As part of providing a psychological service to you, All Psyched Up will collect and record personal information from you that is relevant to your situation, such as your name, contact information, medical history and other relevant information as part of providing psychological services to your child. This collection of personal information will be a necessary part of the psychological assessment and treatment that is conducted.

Our service does not prepare statements, letters or reports for court proceedings.

Collecting, Holding & Exchanging Information

Your personal information is gathered as part of your assessment and treatment, is kept securely and, in the interests of your privacy, used only by your psychologist, and the authorised personnel of the practice (as necessary). Your personal information is retained to document what happens during sessions and enables the psychologist to provide a relevant and informed psychological service to you.

There may be times where it may be helpful for your psychologist to liaise with other people or agencies that are relevant to your child’s goals. If your child utilises a Mental Health Treatment Plan under the Better Access to Mental Health Scheme, then your psychologist must provide summary reports to your GP or Paediatrician regarding their treatment progress. Please note that signing this form acknowledges your consent to allow us to provide these Medicare reports to the referring practitioner.

Access to Client Information

At any stage, a client is entitled to access their personal information kept on file, subject to exceptions in the relevant legislation. A client may view the non-copyrighted material recorded in their or their child’s file upon written request and production of identification. If the client is an adolescent, the client’s consent will be sought before material is accessed by a parent. Adolescent clients who seek to access their file can do so via written request.

Confidentiality

All personal information gathered by the psychologist during the provision of the psychological service will remain confidential except when:

  • it is subpoenaed by a court, or disclosure is otherwise required or authorised by law; or

  • failure to disclose the information would place you or another person at serious risk to life, health, or safety; or

  • your prior approval has been obtained to communicate with another professional agency or another person.

  • When consulting with colleagues, or in the course of supervision, your psychologist will be required to conceal your identity and any associated parties involved; and to preserve your privacy at the utmost professional manner in accordance with the APS Code of Ethics.

Your personal information will not be used, sold, rented, or disclosed for any other purpose.

Telehealth Services

Where appropriate, a service may be provided by telephone or videoconferencing. You are responsible for the costs associated with setting up the technology needed so you can access telehealth services. Your psychologist will be responsible for the cost of the call to you and the cost associated with the platform used to conduct telehealth services. To engage in telehealth consultations, you will need access to a quiet, private space; and the appropriate device, i.e., smartphone, laptop, iPad, computer, a camera, microphone and headphones; and a reliable internet connection.

A telehealth consultation may be subject to limitations such as an unstable network connection which may affect the quality of the psychology session. In addition, there may be some services for which telehealth is not appropriate or effective. Your psychologist will consider and discuss with you the appropriateness of ongoing telehealth sessions.
